# Measure how quickly the next responder can act safely

Handover quality appears in continuity of decisions, not document length. Track missing state, repeated experiments and unowned follow-up work.

## Use a short comprehension check

Ask the incoming owner to identify current impact, active changes, unresolved outcomes and the next decision from the record. Measure the time and missing information during rehearsals.

Do not turn this into a speed contest. The purpose is to expose a summary that requires extensive chat reconstruction before someone can act.

Record which facts were absent or stale. A missing queue-pause owner needs a different correction from an inaccessible dashboard link.

## Track operational discontinuity

Review incidents for repeated failed experiments after handover, accidental reversal of mitigations and missed stakeholder updates. These are signals that state or responsibility did not transfer clearly.

An illustrative handover omits a temporary retry limit. The next shift restores the default and recreates overload. The lesson concerns undocumented configuration state, not whether the outgoing summary contained enough paragraphs.

Separate necessary repeated checks from wasted repetition. Revalidating current health can be appropriate, while rerunning an already disproved destructive hypothesis is not.

## Check ownership of unfinished work

Measure whether temporary changes, uncertain business records and follow-up actions have owners and clear next steps. An incident can be stable while those tasks remain open.

Track overdue reversal conditions and forgotten emergency access. They often indicate that the response record and normal work system were never connected.

Verify handover acceptance rather than assuming a tagged message transferred responsibility. The incoming owner should explicitly acknowledge the current state.

## Use the record to improve the process

Sample incident timelines and compare claimed recovery with business evidence. A dashboard returning to green may precede completion of delayed orders or customer corrections.

Combine those observations with responder feedback about cognitive load and tool access. Avoid measuring word count or update frequency as a proxy for useful communication.

Acceptance should demonstrate that another responder can continue safely and that remaining work stays owned. The record is successful when it prevents ambiguity at a stressful transition, even if it is relatively short.
